Users mysteriously locked out of app?

I’m having trouble with some of our users not having their existing passwords accepted, and then being locked out after three attempts (as I’ve set it up for security purposes - 3 wrong attempts within 5 minutes → 60 min lock out). It’s possible they are mistyping, but odd that it’s just started happening now. The app is not set to require regular password resets.

It seems to be fixed if they use the “forgot password” link to reset password, but I’m worried it will keep happening.

One user appears as “locked” even though she says she didn’t encounter the “incorrect password” scenario.

Is anyone else having this problem?

Never had an issue with this, not had an issue today or recently.
