Lookup field in Add form

Use case:
Tables…
Organization
Contacts
THEY ARE CONNECTED

Add a new record to the Contact table.
Select the Organization from the Dropdown(required field)
When the Organization is selected, lookup fields from the Organization table and immediately display them as read only on the Add Contact add form.

Can anyone help make this work?
