Help with Center Attendance Not Attaching to Operational Data – Only at One Center

Hi All,

I’m experiencing an issue that only seems to be happening at one of our centers (of 5), and I’m hoping someone might have some insight.

Here’s how our system is set up:

  • We have a table called Operational Data, which holds each center’s attendance record for the day.
  • From that, there is a form where users can add Center Attendance, which is attached to a specific Operational Data entry (i.e., they select the participant from a dropdown when submitting the form).
  • Each Center Attendance record includes an attached Participant and the Services they received that day.

The issue:
At only one center, staff are successfully entering center attendance using the form — I can see the records on the backend — but the entries are not being linked to the selected Operational Data entry as expected. This only happens at here, and we’ve tested it in different browsers to rule that out.

My question:
Has anyone else run into an issue where form entries show up in the table but aren’t properly attaching to the related record? Could this be a permissions issue, a form config problem, or something else I’m overlooking?

  • Is your Center Attendance form nested inside an Operational Data page?
  • Are there any record actions that are setting Operational Data back to blank?

To answer your questions:

  • Is your Center Attendance form nested inside an Operational Data page?
    Yes, it is. The form is set up on the Operational Data details page so that any new Center Attendance record is automatically linked to that specific Operational Data entry.
  • Are there any record rules or actions that might be setting the Operational Data field back to blank?
    No, there aren’t any. What’s strange is that this issue only happens occasionally, and later in the same day, it will often start working as expected again — without any changes on our end.
  • Do you have any screenshots or videos you could share?
    I’ll be adding a screenshot showing the Center Attendance form and the table where the entries appear once submitted, to help illustrate the setup.
